15. 13-1378 Resolution approving a modification to the Lake County Tuition Reimbursement Educational Assistance Program. · The County recognizes that the skills and knowledge of its employees are critical to the success of an organization. · In an effort to encourage personal development through formal education, the Lake County Tuition Reimbursement Educational Assistance Program was created. · Acknowledging the substantial increase in education costs, the resolution proposes to increase the amount employees can receive from $1,500 to $2,000 per fiscal year upon successful completion (grade of C or better) of course work. · Currently, employees may be reimbursed up to $500 per course; the recommended policy would eliminate the cap of $500. 16. 13-1379 Resolution approving Alternate Work Arrangements Policy 3.3 that will regulate the processes for employees to work remotely, adjust hours of work, and compress work weeks. · The County recognizes the need to establish procedures that allows qualified employees to work more flexible hours and work in remote locations to enhance productivity. · In an effort to define the circumstances and conditions under which these alternative work arrangements may be authorized, the County developed Policies and Procedures
3.3 Schedule Hours and 3.12 Telework. · Upon review of the fore-stated policies it was determined that they could be combined, a more detailed approve process should be implemented, accountabilities should be established for productivity, and standards established for automation. 17. 13-1384 Resolution approving the modification of policy 10.1 On the Job Employee Behavior. · The County recognizes the need to establish work rules and procedures and to enforce such rules. · In an effort to ensure equitable and consistent enforcement of established work rules, the County developed policy 10.1 On the Job Employee Conduct. · Upon review of the current policy, it was determined that the list of infractions should be expanded, the pre-disciplinary meeting process be described in more detail, and progressive disciplinary be defined with more details. LAW AND JUDICIAL COMMITTEE
18. 13-1367 Joint resolution authorizing a contract with Aramark Correctional Services, Downers Grove, IL, in the estimated amount of $910,304.55 per year for inmate food services. · The County's contract for food services for the Adult Correctional Facility and Community Based Corrections Center expires on December 31, 2013. · A Request for Proposal (RFP) was sent to 11 vendors and proposals were received from three non-local vendors; no local vendors provide this service. · Based on the criteria set forth in the RFP for Inmate Food Services, the proposal submitted by Aramark Correctional Services is the most favorable for Lake County. · Aramark has a large local presence in Lake County which will be helpful to continue operation during the jail kitchen renovation project. · There is a five percent reduction in the cost per inmate meal, which equates to an estimated $16,000 in savings. · The contract authorizes a two-year agreement with three additional one-year renewal options that will cost the County an estimated annual amount of $910,604.55 to be paid from various Lake County accounts.
19. 13-1360 Joint resolution authorizing the State's Attorney's Appellate Prosecutor Program for FY2014 continued participation for a fee of $35,000. · This is a renewal for the continued operation of the State's Attorney's Appellate Prosecutor Program. · Through this program the State's Attorney's Appellate Prosecutor handles Lake County criminal appeals, provides investigative and prosecutorial support in criminal cases, provides comprehensive training programs for prosecutors, and provides monthly case law newsletters, legislative updates, and Uniform Complaint Book supplements. 20. 13-1382 Joint resolution authorizing a two year contract with Kolkmeier Consulting of Springfield, IL for state legislative consulting services. · The County has a need to enter into a contract for Illinois legislative consulting services. · Lake County has a long standing successful relationship with Kolkmeier Consulting of Springfield, IL, since 2004. · Kolkmeier Consulting is extremely familiar with the issues confronting Lake County and has an established rapport with the Lake County legislative delegation in Springfield. · Pursuant to Article 6, Section102 of the Lake County Purchasing Ordinance, the Purchasing Agent consulted with an ad hoc review group that approved a bid exemption on the basis of an existing long standing, successful relationship for professional services and the knowledge gained through this relationship adds value integral to the services provided. · The contract authorizes a two-year agreement with Kolkmeier Consulting, costing the County $50,400 in FY2014 and $51,060 for FY2015.
PUBLIC WORKS AND TRANSPORTATION COMMITTEE
21. 13-1341 Ordinance creating a policy regarding the Automated Meter Reading (AMR) project and amending the rate ordinance to establish associated fees. · On April 9, 2013 the County Board approved a contract with PMI, Inc. for the Water Meter Replacement and Fixed Network AMR System. · The goal of the AMR is to automatically transmit water usage data from the customer's water meter directly to Public Works to facilitate water billing and the availability of usage data for the customer. · Installation began on October 21, 2013 and during the process customers inquired about alternatives to having the transmitter located within the residential structure. · The attached policy was created to provide customers the opportunity to 1) opt out of the AMR program and have their water meter read manually or 2) have the transmitter installed outside. · As these options would require additional services from Public Works, fees are proposed to cover the costs associated with these options. · Amendment of the rate ordinance is required to incorporate these fees. 13-1341 Rate Ordinance AMR Fees.pdf AMR Meter Opt Out Policy (2).pdf Attachments: Lake County Illinois Page 6 of 16
22. 13-1356 Joint resolution awarding a contract for the renovation of the Sign Shop (Building E) at the Lake County Division of Transportation (LCDOT) campus in Libertyville, Illinois, to Camosy Incorporated, Zion, Illinois, in the amount of $166,900, and designated as Section 13-00214-23-MG. · Renovation of the Sign Shop (Building E) at LCDOT: Contract Award. · The Sign Shop supports road maintenance operations. · The renovation of the Sign Shop will reconfigure the sign fabrication area to improve the efficiency of sign production. · The renovation will also include an upgrade to the electrical, HVAC, fire alarm and lighting systems, removal of asbestos, and the installation of a computer workstation for the staff. · Installation and controls for the HVAC system will be funded from the Corporate Capital Improvement Program (CCIP). · The remainder of the improvement is included in the LCDOT 2013 Highway Improvement Program. · A total of three bids were received, ranging from $166,900 to $223,000, and the lowest responsible bidder is Camosy Incorporated, Zion, Illinois, in the amount of $166,900. 23. 13-1357 Joint resolution awarding a contract for the replacement of the Building "B" (Heavy Duty Repair Shop) at the Lake County Division of Transportation (LCDOT) campus in Libertyville, Illinois, to Camosy Incorporated, Zion, Illinois, in the amount of $2,130,800 and designated as Section 11-00214-22-MG. · LCDOT Building "B" (Heavy Duty Repair Shop) Replacement: Contract Award. · The Heavy Duty Repair Shop supports highway maintenance operations. · The Heavy Duty Repair Shop will be replaced to improve the efficiency and reliability of large truck and heavy duty equipment, maintenance and repair operations. · The renovation will also include upgrades to the electrical, HVAC, fire alarm, lighting and equipment lift systems. · The design engineering cost is funded from the Corporate Capital Improvement Program (CCIP). · The remainder of the improvement is included in the LCDOT 2013 Highway Improvement Program. · A total of four bids were received, ranging from $2,130,800 to $2,367,400, and the lowest responsible bidder is Camosy Incorporated, Zion, Illinois, in the amount of $2,130,800. 24. 13-1351 Joint resolution authorizing an agreement with SEECO Consultants, Inc., Tinley Park, Illinois, for geotechnical testing services as part of the Phase III construction engineering services for the proposed roundabout at the intersection of River Road at Roberts Road, at a maximum cost of services described as $150,000, and designated as Section 07-00086-08-CH. 25. 13-1352 Joint resolution authorizing the execution of an agreement with the State of Illinois/Illinois Department of Transportation (IDOT) for the use of federal highway funds for the intersection improvement of River Road at Roberts Road, appropriating $1,855,000 of ¼% Sales Tax for Transportation and Public Safety funds, and designated as Section 07-00086-08-CH. · River Road at Roberts Road, Intersection Roundabout Improvements: IDOT Agreement and Appropriation. · The County has been allocated federal funds to construct a roundabout at the intersection of River Road at Roberts Road, and IDOT administers all federal highway funds. · This agreement provides IDOT will pay approximately 80% of the construction cost and the County will be eligible to receive approximately 80% reimbursement for the construction engineering cost. · This resolution also appropriates $1,855,000 of ¼% Sales Tax for Transportation and Public Safety funds. 26. 13-1353 Joint resolution authorizing an agreement for professional engineering services with Gewalt Hamilton Associates, Inc., Vernon Hills, Illinois, for Phase II design engineering services for the proposed construction of the Deerfield Road bike path, from Thornmeadow Road to Saunders Road, at a maximum cost of services described as $388,405.67, appropriating $470,000 of Matching Tax funds for these engineering services, and designated as Section 99-00038-04-BT. · Deerfield Road Bike Path, from Thornmeadow Road to Saunders Road: Consultant Agreement and Appropriation. · Phase II consulting engineering services will be utilized to prepare plans, specifications, and estimates of costs for this bike path improvement. · The project will consist of constructing a bike path along Deerfield Road, from Thornmeadow Road to Saunders Road, including a portion of elevated boardwalk and associated utility, drainage, and traffic signal work. · The Phase I Engineering for this improvement was completed by the Village of Riverwoods. · Using the Local Government Professional Services Selection Act, the selected and recommended consultant is Gewalt Hamilton Associates, Inc., Vernon Hills, Illinois, at a maximum cost of services of $388,405.67. 27. 13-1354 Joint resolution appropriating a supplemental amount of $15,000 of Matching Tax funds for the Joint Utility Locating Information for Excavators, Inc. (JULIE) ticket screening and locating of Lake County Division of Transportation (LCDOT) storm sewers for excavator dig requests, for various County highways, and designated as Section 13-00000-09-GM. · 2013 JULIE Locates Storm Sewer Services: Supplemental Appropriation. · Pursuant to Illinois law, JULIE serves as a message handling notification service for underground facility owners about planned excavations. · The County's existing storm sewer locating contract with USIC Locating Services, Inc., Indianapolis, Indiana, began on May 15, 2012 and allows for a total of four, one year contract extensions. · This County Board at its meeting of November 20, 2012, approved $95,000 for the January 1, 2013, thru December 31, 2013, contract period. · The number of JULIE ticket screening and locate requests will exceed our 2013 estimate due to increased construction activity. · This resolution is appropriating a supplemental amount of $15,000 for the 2013 contract period.
28. 13-1355 Joint resolution appropriating $115,000 of Matching Tax funds for the Joint Utility Locating Information for Excavators, Inc. (JULIE) ticket screening and locating of Lake County Division of Transportation (LCDOT) storm sewers for excavator dig requests for various County highways, and designated as Section 14-00000-09-GM. · 2014 JULIE Locates Storm Sewer Services: Appropriation. · Pursuant to Illinois law, JULIE serves as a message handling notification service for underground facility owners about planned excavations. · The County's existing storm sewer locating contract with USIC Locating Services, Inc., Indianapolis, Indiana, began on May 15, 2012 and permits a total of four one year contract extensions. · The contract will be extended for a year to cover the time period January 1, 2014, to December 31, 2014. · Prior to the formal extension being granted, an appropriation of $115,000 of Matching Tax funds is required.
29. 13-1381 Joint resolution appropriating a supplemental amount of $100,000 of 2011A Tax Exempt General Obligation (GO) Bonds Road Construction funds for the acquisition of necessary right-of-way, by agreement or condemnation, and other right-of-way costs associated with the RR Underpass Washington Street Thoroughfare, Washington Street from Haryan Way to Lake Street, and a grade separation of the Canadian National Railroad, and designated as Section 11-00121-11-BR. · Railroad Underpass, Washington Street Thoroughfare Improvement; Right-of-Way Acquisition: Supplemental Appropriation with Condemnation Authority. PLANNING, BUILDING AND ZONING COMMITTEE
30. 13-1338 Ordinance to amend the Lake County Unified Development Ordinance (UDO).
· On October 1, 2013, staff presented an initial review of proposed text amendments to
Article 10 - Subdivisions and Article 11 - School and Park Contributions of the Unified
Development Ordinance (UDO).
· The proposed amendments are designed to delegate subdivision-related decisions to
staff to further streamline the process for subdivisions and address internal
inconsistencies.
· The attached Exhibit A incorporates the original staff recommended amendments and
revisions recommended by the Regional Planning Commission (by a vote of 8 to 0) at
its October 15 meeting.
· In accordance with state statute, the Planning, Building and Zoning Committee
conducted a public hearing on text amendments related to subdivisions and school and
park contributions on December 3, 2013.

31. 10-0994 Board of Health Report.
32. 13-1363 Joint resolution authorizing an emergency appropriation in FY13 Health Department
budget in the amount of $99,180 from the Illinois Department of Healthcare and Family
Services.
· The Lake County Health Department and Community Health Center has received funds
from the Illinois Department of Healthcare and Family Services.
· Funds result from an incentive payment to eligible Medicaid providers to provide
comprehensive, preventive medical care to children to ensure they receive all
recommended well care visits each year.

33. 13-1365 Joint resolution authorizing an emergency appropriation in the FY13 Health
Department budget in the amount of $14,414 from the Illinois Department of Public
Health.
· The Illinois Department of Public Health has increased the FY14 Tobacco Free
Communities (TFLC) grant award by $14,414.
· The total amount of the grant is $289,297, the difference having been budgeted in
FY14. · An emergency appropriation in the amount of $14,414 is requested to be appropriated in FY13 and will be used for advertising costs. Attachments: Emergency Appropriation for FY13 November 2013 - IDPH
LAW AND JUDICIAL COMMITTEE
34. 13-1369 Joint resolution authorizing an emergency appropriation in the Coroner Fees Fund in
the amount of $16,136 for operational supplies, clothing and uniforms, training, and
cell phone expenses.
· The Lake County Coroner's Office receives revenue from cremation services and
requests for records.
· The revenue received is deposited into the Coroner Fees Fund and used for
equipment, supplies and expenses. In FY13, $105,000 in revenue was budgeted. To
date $121,136 has been received.
· An emergency appropriation in the amount of $16,136 is needed to recognize this
additional revenue.
· Additional funds are needed for uniforms and equipment for the deputies, training and
network extenders to increase cell phone coverage in the field.
35. 13-1328 Joint resolution authorizing an emergency appropriation and acceptance of a grant
award in the amount of $8,800 for a Tobacco Enforcement program from the State of
Illinois Liquor Control Commission.
· This is the annual Tobacco Enforcement Grant which covers the time period of August
1, 2013 through June 30, 2014.
· The total grant of $8,800 covers all expenses related to enforcement including: 141

36. 13-1364 Joint resolution authorizing an emergency appropriation in the FY14 budget, with
offsetting revenue, in the amount of $60,000 to implement Juvenile Accountability
Block Grant programming for the Nineteenth Judicial Circuit.
· The Illinois Department of Human Services recently announced that the Nineteenth
Judicial Circuit was awarded $54,000 in Juvenile Accountability Block Grant funds to
continue the juvenile probation pre-employment program that began in late 2009. The
agreement runs from December 1, 2013 through June 30, 2014.
· The Nineteenth Judicial Circuit will provide a 10 percent match of $6,000 from the
Probation Services Fee Fund.
· The funding will be used to continue funding a part-time position to serve as program
coordinator, to provide a job readiness curriculum to approximately 35 youth and to
contract with local agencies and employers to provide approximately 18 youth with paid
internships. Supplies and bus passes are also included in the budget.
· This resolution authorizes an emergency appropriation of $60,000 and the execution of
a program agreement to accept the grant funding.

37. 13-1368 Joint resolution to enter into a contract with Wexford Health Services, Inc., Pittsburgh,
PA, in the estimated amount of $2,241,265 per year for inmate and juvenile healthcare
services and transfer $300,000 in anticipated savings to General Operating Expense
Contingency.
· The current contract to provide healthcare services for the Adult Correctional Facility
and Juvenile Detention Center will expire on January 31, 2014.
· A Request for Proposal (RFP) was developed following extensive research into
contracts and best management practices for healthcare services provided to other
comparable county and juvenile detention facilities.
· The RFP was sent to 34 vendors and proposals were received from four non-local
vendors; no local vendors provide this service.
· An interdepartmental committee comprised of representatives from the Sheriff's Office,
Court Administration, Juvenile Detention, Health Department, Risk, Purchasing, and an
independent third party subject matter expert evaluated the proposals.
· Based on the criteria set forth in the RFP for Inmate and Juvenile Healthcare Services,
the proposal submitted by Wexford Health Services Inc. is the most qualified and most
favorable for Lake County.
· In addition to healthcare services, the contract will provide the Adult Correctional
Facility with a comprehensive, non-proprietary electronic medical record solution
(CorEMR) and a telemedicine option.
· The contract authorizes a two-year agreement with three additional one-year renewal
options that will cost the County an estimated annual amount of $2,241,265.

38. 13-1376 Ordinance approving the Admissions Agreement with the Central Lake County Joint
Action Water Agency (CLCJAWA) to allow for two new water service areas and
authorizing all necessary acts.
· The County Board established Special Service Area Number 16 at the board meeting
of November 12, 2013 and authorized the issuance of bonds in an amount not to
exceed $46,000,000 for the purpose of funding the construction of improvements and
extensions to the delivery system of the CLCJAWA to provide Lake Michigan water to
the SSA area.
· This ordinance finds and determines that it is necessary and desirable that the County
obtain water service from CLCJAWA for the Fox Lake Hills and Grandwood Park
Service Areas, subject to the terms and conditions of the Admissions Agreement.
· By this ordinance, the chairman of the County Board is authorized and directed to
execute and deliver, and the County Clerk is authorized to attest and seal the
Admissions Agreement.

39. 13-1331 Ordinance providing for the issuance of $7,000,000 Special Service Area (SSA)
Number 16 Ad Valorem Special Tax Bonds for the financing of the Lake Michigan
Water Project, and providing for the levy and collection of an annual tax for the
payment of the principal and interest on said bonds.
· The County Board established SSA Number 16 at the board meeting of November 12,
2013 and authorized the issuance of bonds in an amount not to exceed $46,000,000 for
the purpose of funding the construction of improvements and extensions to the delivery
system of the Central Lake County Joint Action Water Agency (CLCJAWA) to provide
Lake Michigan water to the SSA area.
· This ordinance provides for the issuance of $7,000,000 of Ad Velorem Special Tax
Bonds to fund design engineering, purchase of rights of way, and other construction
costs for initial project work efforts.
· Approval of this ordinance by roll call vote is requested.

Mancini, Inc. in the amount of $1,783,030 and to authorize an emergency appropriation in FY 2014 of $1,800,000 to make funds available for the Diamond/Sylvan Lake Diversion Phase III Project. 41. 13-1350 Joint resolution authorizing an emergency appropriation of $34,421,764.55 in the 2013
Road Construction Bond fund for road improvements to public roads within the County,
including Rollins Road at Illinois Route 83, and Fairfield Road at Illinois Route 176, and
designated as Section 13-00999-50-GB.
· Lake County 2013 Road Construction Bond Fund: Construction Appropriation.
· Lake County Board, at its meeting of March 15, 2011, approved the issuance of
General Obligation Bonds (Sales Tax Alternate Revenue Source).
· The County has issued the bonds to pay for road improvements to public roads within
the County, including Rollins Road at Illinois Route 83, and Fairfield Road at Illinois
Route 176.
· It is now necessary to appropriate the budget funds in a newly created Road
Construction Bond fund, and also to budget for the revenues generated by the receipt
of the bond proceeds.
· This resolution authorizes an emergency appropriation of $34,421,764.55 in the 2013
Road Construction Bond fund for road improvements to public roads within the County,
including Rollins Road at Illinois Route 83, and Fairfield Road at Illinois Route 176, and
designated as Section 13-00999-50-GB.
42. 13-1358 Joint resolution authorizing the renewal of two agreements between Lake County and
Pace Suburban Bus Division of the Regional Transportation Authority (RTA) providing
terms and conditions for the year of 2014, by which Pace will coordinate operations of,
and partially fund, the Lake County Northwest Demonstration Project for the expansion
of the paratransit services to meet the needs of the mobility challenged constituent
populations in Antioch, Avon, Fremont, Grant, Lake Villa and Wauconda Townships.
This project is designated as Section 09-00280-01-ES.

· Transfers in various funds between line items in the FY 2013 budget are needed to
align expenses with correct accounts.
· The County Board must approve line item transfers under certain conditions.
· A listing of the various transfers is attached to the resolution.

45. 13-1340 Resolution authorizing emergency appropriations for FY 2014 in various funds for
certain projects, items and activities, which were budgeted in the prior year and not
completed.
· This resolution authorizes the carry-over, or re-authorization, of certain FY 2013
activities.
· These activities were not included in the FY 2014 budget while the carry-over list and
associated budget amounts were determined; to include them would have hindered the
proper scrutiny of the new FY 2014 budget.
· Without this action, these projects and/or items will not have the budget authority
required.
